    1. Patient Information

Who is this enquiry for? *

    2. How Did You Lose Your Leg?

The cause of your amputation can sometimes affect how your prosthesis may be funded. Some patients may have access to third-party or compensation funding even if they do not have medical aid.

    Road Accident Funding

Was a Road Accident Fund (RAF) claim opened following the accident? *

Has RAF previously made a payment to you in relation to this claim? *

    Claim identification

If RAF has previously made a payment to you and you do not have your RAF claim number available, you may provide your South African ID number to assist LAOS in identifying the claim.

Your ID number will only be used by LAOS for purposes related to verifying or following up on the RAF funding pathway for this enquiry.

    Workplace Injury Funding

Did the injury happen while you were performing your work duties? *

Was the injury reported as an injury on duty? *

Your ID number may be used by LAOS to assist in verifying whether an occupational injury claim was registered and to identify the applicable compensation pathway.

    3. Funding

You May Have Another Funding Pathway

Based on the information you provided, there may be a funding or compensation pathway worth investigating before you consider paying privately.

This does not mean that funding has been approved. LAOS will need to review your circumstances and any available claim or funding information.

    4. Medical Scheme / Medical Aid Details

Please provide the patient's medical scheme details. LAOS will use this information to assess the appropriate authorisation pathway.

Is the patient the main member of the medical scheme? *

Providing medical scheme details does not guarantee authorisation or payment. LAOS will review the information and advise you on the next steps.

    4. Private / Cash Patients

    Below-Knee Prosthesis Pricing

Our private below-knee prosthetic solutions start from approximately R80,000 and can exceed R150,000, depending on the prosthetic solution recommended following your clinical assessment.

Your final price will depend on your clinical requirements, functional goals, prosthetic design and components selected.

    10% Upfront Settlement Discount

A 10% discount may apply when the full quoted amount is settled upfront.

The upfront settlement discount does not apply to the 50% deposit arrangement or long-term monthly payment options.

    5. Payment Method

If your final quotation falls within your selected range, how would you prefer to pay? *

    6. Long-Term Payment Interest

LAOS is exploring whether selected prosthetic solutions could become accessible through longer-term monthly payments in partnership with suitable finance providers.

This option is currently being explored and is not currently an offer of finance.

Your responses will help us understand demand for such a service.

Would the payer expect to have a stable source of income throughout the proposed payment period? *

This enquiry is for market-research and patient-planning purposes. It is not a credit application, financing agreement, approval or guarantee that finance will be offered.

    7. Financial Assistance

We understand that the cost of a prosthesis may not be within everyone's financial reach.

LAOS maintains a Patient Support Register to document patients who require financial assistance and the rehabilitation devices they need.

Registration does not guarantee sponsorship, funding or provision of a prosthesis.

After submitting this enquiry, you may be directed to our Patient Support Register.
